Overseas Vietnamese residing in the US with damaged Vietnam passport or without a Vietnamese passport (due to loss) can apply for passport replacement in person or by mail.
For those who have never had a Vietnam passport:
Record in the birth registration book
Children born in the US to Vietnamese parents (who have valid Vietnam passport); to parents, one of whom is not Vietnamese and whose birth certificates have been issued by the US relevant authority, can apply for Vietnamese birth certificate and passport.
b. Birth registration:
Children born in Uganda to Vietnamese parents; to parents one of whom is not Vietnamese and do not have a Ugandan birth certificate can register for birth certificate and apply for new passport.
Adopted children can keep their citizenship by renewing their Vietnamese passports.
INSTRUCTIONS FOR FILLING IN THE APPLICATION FORM FOR PASSPORT RENEWAL:
NUMBER 1: FULL NAME (ORIGINAL VIETNAMESE NAME)
NUMBER 3: DATE OF BIRTH
NUMBER 6, 7: ADDRESS IN THE USA, PHONE AND EMAIL
NUMBER 8: ADDRESS IN VIETNAM BEFORE THE CHILD LEAVE FOR THE US
NUMBER 10: FATHER’S NAME, MOTHER’S NAME (ADOPTIVE PARENTS)
NUMBER 12: REQUEST TO RENEW PASSPORT
NUMBER 14: NATIONAL ID/ UGANDA PASSPORT NUMBER
ADOPTIVE PARENTS CAN SIGN THE FORM ON BEHALF OF THEIR CHILD
IT IS NOT REQUESTED TO FILL IN THE REMAINING OF THE FORM.
IF YOU WANT TO HAVE YOUR CHILD’S NEW UGANDAN NAME IN THE NEW PASSPSORT, WE CAN PRINT THE NEW NAME ON PAGE 7 OF THE NEW VIETNAM PASSPORT. YOU NEED TO PROVIDE A COPY OF THE PETITION OF NAME CHANGE (OR ANY PAPER SHOWING THAT THE ONE IN VIETNAM NAME AND THE ONE IN UGANDAN NAME IS THE SAME.
